29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: On or about August 21, 2013, and at times prior thereto, the employer had not trained a laborer using an aerial lift to recognize hazards associated with aerial lifts and the procedures to control those hazards, exposing the employee to the hazard of falling and other potential hazards associated with the operation of it.

On or about August 21, 2013, and at times prior thereto, the employer did not ensure that employees were not exposed to an approximate 18 foot fall to the ground when performing plastering operations near an open side of a canopy platform, exposing employees to the hazards of falls.
