29 CFR 1926.24: The employer did not develop and/or maintain an effective fire protection and prevention program at the job site throughout all phases of the construction, repair, alteration or demolition work and/or did not ensure the availability of the fire protection and suppression equipment required by Subpart F of this part: (a) West Campus, PT Building Stair B - A fire exit discharge did not offer a usable means of egrees to clear the building. The area was below grade and covered by a grid of rebar in all directions; on or about 08/20/13.
