29 CFR 1910.38(f)(3): The employer did not review the emergency action plan with each employee covered by the plan when the plan was changed: This violation occurred at the BEI-Hawaii Hilo warehouse on or about August 21, 2013 and at times prior thereto; where the employer did not ensure that the emergency action plan was reviewed with each employed covered by the plan when it had changed. The identified exposed employees to inhalation hazards and the changes to the plan include but are not limited to: a. The facility no longer storing chlorine emergency kits A & B. b. Installation of a chlorine and sulfur dioxide detector and alarm.
