29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. On or about 08/20/2013 at the job site, a scaffold system two frames high was being used to apply stucco on the building wall, no guardrail were installed exposing employees to a 18 ft. fall hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) On or about 08-20-2013 in front of building #22544, where the building walls were being stuccoed, the use of fall protection was not enforced, exposing an employee to a 13 ft. fall hazard.
