29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:     a) At B R Concrete - the employer did not provide adequate safety training, in that, information was not provided to ensure employees understood the rules and regulations or how to recognize and avoid unsafe conditions. No training was provided regarding the safe operation and use of a STIHL Concrete Saw model TS 420, s/n  174459496, observed on or about August 21, 2013.
