29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system. On or about  August 21, 2013, and at times prior thereto, the employer did not ensure that each employee working at elevated locations 6 feet or more above a lower level were protected by guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about August 21, 2013 , and at times prior thereto, employees working on the commercial  property under renovation, had not received training on the use of fall protection prior to being allowed to work on the roof, exposing the employees to fall hazard of up to 18 feet.
