29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about 07/31/2013, at the old Shell Oil Field, located in Bayou Sorrell, Louisiana, the employer did not provide and ensure the use of protectine equipment including eye and face protection, hand protection, foot protection, and flame-retardent treated clothing (FRC) were used as necessary to protect employees from burns due to potential flash fires. The employees were cutting into a abandoned gas well flow line when the incident occurred.

29 CFR 1910.252(a)(2)(iv): The individual responsible for authorizing cutting or welding operations did not designate precautions to be followed in granting authorization to proceed preferably in the form of a written permit: On or about 07/31/2013, at the Old Shell Oil Field, located in Bayou Sorrell, Louisiana, the employer allowed employees to cut into a abandonded three inch oil and gas pipeline without any verbal or written precautions given to the employees prior to the pipeline being cut.
