Jobsite: No daily or weekly inspections being conducted by a competent person on a regular basis on site to identify missing handrails, unsafe use of step ladders and fall hazards.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. Jobsite: Employees were exposed to Slips Trips and Falls (mssing handrails in stairways) due to lack of awareness training from the competent person on site due to not installing handrails in stairways (Building 2). Also employees exposed to falls next to window openings when employees were using a step ladder incorrectly.

If such modifications or changes are made, the capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. In no case shall the original safety factor of the equipment be reduced.
