29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:. At the job site, the employer failed to ensure that exposed employees were provided and used fall protection. These employees were installing copper upon a low-slope roof that measured approximately 11 feet 6 inches ground to eave height.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At the job site employees were utilizing a portable extension ladder to gain access to the roof. The side rails belonging to this ladder extended approximately nine inches above the eave. Employees using this ladder to descend were exposed to a serious fall hazard.
