29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed: a. 20 Oak Street, Jackson, GA (Bathroom) - Employees were exposed to health hazards. The only restroom in the store which the employees access several times a day, is kept in an unsanitary condition. There was black mold accumulating on several areas of the bathroom walls due to water leaking through the bathroom walls, on or about August 20, 2013.

29 CFR 1910.305(b)(1): Unused openings in boxes, cabinets, or fittings were not effectively closed: a. 20 Oak Street, Jackson, GA (Rack Storage Room) - Employees were exposed to an electrical hazard. The live 240V circuit breaker electrical panelboard was not equipped with blanks on the unused openings, exposing employees to live parts when accessing the circuit breaker box to turn on and off the shop lights on a daily basis, on or about August 20, 2013.
