29 CFR 1910.134(c)(2)(i): When the employer determined that any voluntary respirator use was permissible, the employer did not provide the respirator users with the information in Appendix D of this section ("Information for Employees Using Respirator When Not Required Under the Standard"): (Construction Reference 1926.103)    Hangar 323, First Floor: On or about 13AUG13, the employer permitted an employee to provide his own respirator, and voluntarily use the respirator without evaluating whether the employee was medically able to use the respirator, kept the respirator clean and maintained, and did not create a health hazard to the employee.

29 CFR 1926.1060(b): The employer did not provide retraining as necessary for each employee using ladders and stairways so that the employee maintained the understanding and knowledge acquired through prior training:  Hangar 323, First Floor: On or about 13AUG13, the employer was notified of its employees improper use of ladders on the worksite twice in one day by the owner of the worksite; the employer did not provide retraining to the employees to maintain the employees' understanding of the proper use of the ladders provided by the employer.
