Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to fall hazards: Warehouse: On, or about, August 22, 2013, retractable lanyards provided on the order picker trucks were not inspected on a regular basis in accordance with manufacturer's recommendations and industry standards. At the time of the inspection, two of the lanyards did not appear to be working properly.
