29 CFR 1910.141(a)(5): A continuing and effective extermination program was not instituted where rodents, insects, or other vermin were detected:   a)  Colonial Creek Campground Amphitheater, building. 1054;  There was mouse droppings inside of the building, and a mouse or pack rat nest in the back storage closet.   The National Park Service was previously cited for a violation of this standard which was contained in OSHA inspection no. 902964 citation 1, item 1, issued on May 29, 2013 with respect to a workplace at 29 Palms, California.     The National Park Service was previously cited for a violation of this standard which was contained in OSHA inspection no. 540940 citation 1, item 4, issued on August 24, 2012 with respect to a workplace at Whiskeytown, California.    The National Park Service was previously cited for a violation of this standard which was contained in OSHA inspection no. 313533580 citation 3, item 3b, issued on May 3, 2011 with respect to a workplace at San Juan, Puerto Rico.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:   a)  North Colonial Creek Campground Pumphouse;  Employees are working with Haza Multi-chlor sodium hypochlorite solution and there is not an eyewash station in the work area, exposing employees to possible permanent eye damage from a liquid corrosive.    The National Park Service was previously cited for a violation of this standard which was contained in OSHA inspection no. 899100 citation 1, item 3, issued on May 31, 2013 with respect to a workplace at Ventura, California     The National Park Service was previously cited for a violation of this standard which was contained in OSHA inspection no. 314899287 citation 1, item 1, issued on June 8, 2011 with respect to a workplace at Sedro Woolley, Washington.      The National Park Service was previously cited for a violation of this standard which was contained in OSHA inspection no. 313533580 citation 1, item 6, issued on May 3, 2011 with respect to a workplace at San Juan, Puerto Rico.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:   a)  North Colonial Campground Pumphouse;  A thorough PPE evaluation had not been performed for the water treatment operator, to ensure they wore chemical goggles, a face shield, gloves and an apron when adding Haza multi-chlor sodium hypochlorite to the water treatment system to prevent chemical burns on the skin and eyes.
