29 CFR 1910.119(j)(2): The employer did not establish written procedures to maintain the on-going integrity of process equipment. The employer does not ensure that written procedures are established to address the inspection, testing, and preventative maintenance of the re-boilers and fire-heaters in the plant. This condition was observed on or about August 20, 2013 in the gas plant for reboilers and fire heaters, exposing employees to fire and explosion hazards resulting from the uncontrolled release of flammables into the atmosphere.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. On or about August 20, 2013, in the Amine Regeneration Unit Northside of the plant employees were working around Amine without suitable facilites for quick drenching or flushing of eyes, exposing employees to chemical burn hazards.
