29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate:      a) On or about August 9, 2013, employees in the facility were exposed to possible ceiling collapse hazards in that materials were stored in overhead areas above occuppied offices and other rooms.  The load capacities of these overhead storage areas were not marked.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):       a) On or about August 3, 2013, employees in the production area of the plant using the manual sheer were exposed to possible cuts and/or amputation type injuries in that the point of operation (the shearing blade) was not adequately guarded to prevent an employee's hand or fingers from entering the point of operation.  An employee did amputate the tip of their left index finger when operating the machine on August 3rd.
