29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years:    a.  At the work site located at 2135 James Parkway #3, Heath, Ohio, employees used propane powered industrial vehicles to pick and load customer orders for commercial brake parts that had been palletized and stored according to type.  The employer had not completed evaluations on the operators of the powered industrial trucks every three years as required by the standard.
