29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D).  a) Baler:  On or about August 21, 2013 and at times prior, the employer exposed his employees to amputation, caught-in and electrical hazards while not being provided with specific lockout procedures for the Baler machine.  b) Shear:  On or about August 21, 2013 and at times prior, the employer exposed his employees to amputation, caught-in and electrical hazards while not being provided with specific lockout procedures for the Shear.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).   a)   On or about August 21, 2013 and at times prior; the employer exposed employees to struck-by hazards in that employees were allowed to operate a powered industrial truck without the employer first ensuring the employees were competent to operate the equipment as demonstrated by the completion of the required training and evaluation specified in the regulation.     Panhandle Iron & Scrap was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(1)(i), which was contained in OSHA inspection number 315111625, citation number 1 item 1a and was affirmed as a final order on 12/16/2010, with respect to a workplace located at 1004 N Beverlye Rd. Dothan, AL 36303.

Panhandle Iron & Scrap was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(h)(1), which was contained in OSHA inspection number 315111625 citation number 1item number 3b and was affirmed as a final order on12/16/2010 with respect to a workplace located at 1004 N Beverlye Rd Dothan AL, 36303.
