29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):    (a)  At Stewart-Morrison Redi-Mix:  The  employer did not perform an evaluation of noise and other possible air contaminants such as silica, respirable dust and total dust to determine the type of respiratory protection and other personal protective equipment such as hearing protection necessary to perfom the work safely.

29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146    (a) At Stewart-Morrison ready mix: The employer failed to train the employees in confined spaces.  Employees were entering truck mixers without being trained on the confined spaces hazards.
