29 CFR 1926.405(g)(2)(iii): The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used: On August 21, 2013 employees at the job site at or near 825 South Maguire Street in Warrensburg, MO were exposed to electrical hazard in that their flexible cord was not repaired so that the splice retained the outer sheath properties and usage characteristics.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On August 21, 2013 at the jobsite located at or near 825 S Maguire Street in Warrensburg, MO, employees were exposed to a fall hazard in that they were working on a low sloped roof, over 6 foot above lower level without an approved form of fall protection.
