29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  On or about August 20, 2013, the distance between the grinding wheel periphery and the adjustable work rest on the Porter Cable grinder used by Maintenance employees had a gap between the wheel and work rest that measured approximately five-eighths (5/8)  inch, exposing employees to the hazard of being caught in the rotating wheel.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  On or about August 20, 2013, the distance between the grinding wheel periphery and the adjustable tongue guard on the Porter Cable grinder used by Maintenance employees had a gap between the wheel and tongue guard that measured approximately one inch, exposing employees to the hazard of being struck by flying debris in the event of wheel breakage.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  On or about August 20, 2013, the junction box covers were missing on the following boxes, exposing employees to an electrical hazard:    a:  The box containing the SHURflo, model 4UN54, pump for the steam cleaner was missing the box cover, exposing bare conductors.    b:  At the junction box for the fuel shutoff control, the cover was missing in the maintenance area.

29 CFR 1910.305(j)(2)(iv): A receptacle installed in a wet or damp location was not suitable for the location:  On or about August 20, 2013, electrical outlets were used in wet locations without a ground fault interruption circuit (GFCI) at either outlet or the breaker panel in the following instances, exposing employees to an electrical hazard:    a:  The receptacle box in the maintenance area powering the water fountain next to the sink was not a GFCI outlet.      b:  A surge protector was used in the truck wash area to power a radio.  The cord for the surge protector ran through the wall and into an outlet in another room.  Neither the outlet nor the surge protector had a GFCI.
