29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. Guardrail systems were not installed before the scaffold is released for use by employees other than erection/dismantling crews: On or about August 21, 2013; employee midway of the second floor balcony of the apartment building #2 was exposed to a fall greater than 10 feet to the ground level below while installing soffit.

29 CFR 1926.451(b)(5)(ii): Each platform greater than 10 feet in length shall not extend over its support more than 18 inches (46 cm), unless it is designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end: On or about August 21, 2013 employee was exposed to a fall greater than 10 feet in elevation to the ground level below from a shore scaffold when employee was installing soffit beneath the fascia on the 2nd floor balcony building #2 and standing on the plank that extended approximately three feet beyond its support.
