29 CFR 1910.27(d)(2): Fixed ladder(s), used to ascend to heights exceeding 20 feet and equipped with cages or wells, were not provided with landing platform(s) for each 30 feet of height or fraction thereof. a) On or about August 01, 2013 employees climbed the East Load out leg ladder that did not have offsets from adjacent sections and was not equipped with landing platforms at the required intervals, exposing employees to fall hazards. b) On or about August 01, 2013 employees climbed the East Dump Pit leg ladder that did not have offsets from adjacent sections and was not equipped with landing platforms at the required intervals, exposing employees to fall hazards. Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).

Facility: On or about 19 August 2013 employees used electrical panels for the delivery and movement of grain. Panels were not labeled with the appropriate voltage markings. Exposing employees to a shock hazard. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
