29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) TEV Roofing, Inc., Bowling Green, Ohio:  On or about August 21, 2013, at address 1133 Oriole Dr., Bowling Green, Ohio, the employer did not ensure that employees were protected from falls while installing new roofing materials to a steep sloped roof (6:12) on a residential type structure.  An employee was exposed to a potential fall of approximately 19.7 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) TEV Roofing, Inc., Bowling Green, Ohio:  On or about August 21, 2013, at address 1133 oriole Dr., Bowling Green, Ohio, the employer did not ensure that all employees exposed to falls had been trained in the appropriate use of fall protection, such as, but not limited to the use of personal fall arrest systems.  An employee was exposed to falls of approximately 9.6 feet to the ground below, which could result in permanent disability or death.
