29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: (a) Luz Electric, 275 S. Cherokee St., Denver, CO 80203: On and before May 21, 2013 the employer did not make sure the employee had attached the lanyard to the D-ring attachment point in the center of the wearer's back. The employee had attached the lanyard to the D-ring on the waist side of the harness. This condition exposed the employee to injuries in the event of a fall.
