29 CFR 1926.651(b)(3): When the excavation operations approach the estimated location of underground installations, the employer did not determine the exact location of the installations by a safe and acceptable means: (a) Ames-Granite A Joint Venture at U.S. Highway 36 & S. 88th St., Louisville, CO 80027: On and before July 17, 2013 the employer did not determine the exact location or use a safe and acceptable means to find underground utilities in that, an employee of Ames-Granite using a Caterpillar 330 Track Excavator struck an underground natural gas line. The natural gas line ruptured and forced the evacuation of the construction site. This condition exposed employees to potential fire or explosion hazards.
