29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a. On or about 8/9/13, fabrication area, the Cincinnati 230 hydraulic press brake did not have guarding at the sides of the machine, the palm controls could not be selected for use, and the safe distance of the light curtain from the point of operation was not calculated according to the manufacturer's installation instructions in that the floating blank selector did not accurately indicate the number of light curtain beams floating thereby exposing employees who operated the press brake to amputation hazards.
