29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite. On or about August 19, 2013, east side of the commercial building, the employer did not provide a fire extinguisher for the tar kettle and flammable material utilized to heat the kettle exposing employees to fire hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about August 19, 2013, on the east side of the commercial building, employees who worked without fall protection were exposed to a fall hazard of approximately 16 feet while engaged in roofing activities on a low slope roof.
