OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to combustible paper dust explosion and deflagration hazards while working at or near a filter media-type dust collector (baghouse type air-material separator) system which was not adequately designed to prevent or minimize employee exposure in the event of an internal deflagration:    (a)  On or about August 26, 2013 the indoor AVSC filter media-type dust collector (baghouse type air-material separator) located inside the facility near the shipping area lacked means of 1) explosion protection and 2) deflagration propagation protection (isolation) to the upstream dirty-air inlet duct and associated processes, the downstream clean-air exhaust, and the unit's discharge hopper rotary airlock valve.
