Industries electrical protective rubber gloves, used by maintenance employees during electrical troubleshooting and replacement of a fuse in the electrical panel 1P (Type LX 2353, Serial # PRPFP231217-06), were tested at least once every six months. Employees were exposed to 480 volts electrical hazards.      B)     Pressed Paperboard Technologies, LLC, located at 115 Bentley Ct., Findlay, OH 45840: On or about September 2, 2013, the employer did not ensure that Salisbury electrical protective rubber gloves, used by the maintenance employees during installation of the hand dryer's breaker in the electrical panel, were tested at least once every six months. Employees were exposed to 480 volts electrical hazards.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Employees did not isolate all energy sources in order to control hazards associated with rotating parts of the machine while performing activities such as, but not limited to, changing dies and cleaning Anilox rollers. Employees were exposed to amputation hazards.      B)     Pressed Paperboard Technologies, LLC, located at 115 Bentley Ct., Findlay, OH 45840: On or about August 26, 2013, the employer failed to develop machine-specific energy control procedures to control hazardous energy of the Bridgeport milling machine (Model F-7347-00-248, Serial # 1-18-0171) located in the facility's maintenance department.  Employees did not isolate the electrical energy source in order to control hazards associated with rotating parts of the machine while performing activities such as, but not limited to, changing drill bits. Employees were exposed to injury from unexpected start-up and energy release. B)     Pressed Paperboard Technologies, LLC, located at 115 Bentley Ct., Findlay, OH 45840: On or about August 26, 2013, the employer failed to ensure that the Grizzly milling & drilling machine (Model G1006, Serial # 3519), located in the facility's maintenance department, was properly guarded in order to prevent employees from inadvertently being caught by the rotating chuck during the machine's operation.
