29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:      a)  Bent Edge Line, Building 3, 300 Halstead Blvd, Zelienople, PA 16063:  Observed on or about July 18, 2013, the employer did not provide appropriate gloves to employees working in close proximity infrared heaters which reached temperatures of approximately 1000 degrees Fahrenheit.  Employees working on the Bent Edge machine were exposed to burn hazards to their fingers, hands, and arms while operating and removing jams from the Bent Edge machine.

Employees were exposed to electric shock and caught-by hazards while changing fuses on high voltage equipment and removing jams from the Bent Edge machine.

29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) thru (c)(4):  a) Shop Area, Building 3, 300 Halstead Blvd, Zelienople, PA 16063:  Observed on or about July 18, 2013, an Makita brand angle grinder (Model Number 9564CV, Serial Number 191809A) equipped with a Type 47, 4 and 1/2 inch diameter cut off wheel (Part Number A60S-BF) was not equipped with a safety guard.  Employees using the angle grinder were exposed to cut-by hazards.

29 CFR 1910.303(g)(2)(i):  Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures:   a)  Bent Edge Line, Building 3, 300 Halstead Blvd, Zelienople, PA 16063:  Observed on or about July 18, 2013, an electrical cabinet containing high voltage (480 Volt) power was left open.  A portable fan was placed in front of the open cabinet to provide cooling to the cabinet.  Bent Edge machine operators were exposed to the hazard of electric shock while accessing the control switches/buttons on the open door of the electrical cabinet.

29 CFR 1910.305(g)(1)(iii)(A):  Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    a)  Weld Tester Machine, Building 3, 300 Halstead Blvd, Zelienople, PA 16063:  Observed on or about July 18, 2013, a flexible cord was being used as a substitute for the fixed wiring of the building.  The cord was used to supply power to a Weld Tester/Pull Tester machine.  The cord crossed over the floor of a walkway and under equipment located behind the Weld Tester machine.   Employees using the Weld Testing Machine or walking over the cable were exposed to electric shock and tripping hazards.
