29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Throughout the facility - A lockout tagout was not was not written or implemented, consisting of machine specific procedures, training for the affected employees and periodic inspections. Employees were exposed to amputations, and compound fractures while working near equipment that is not de-energized before conducting maintenance, changing belts, lubricating and replacing parts for equipment such as but not limited to, conveyors, aggregate bins, and the mixer. Basic Ready Mix was previously cited for a violation of this occupational safety and health standard , which was contained in OSHA inspection number 314645565, citation number 1, item number 4 and was affirmed as a final order on December 06, 2011, with respect to a workplace located at 712 N. Wall Street, Calhoun, Georgia.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. Truck washing area - an eyewash station was not available for employees using corrosive chemicals, such as, but not limited to APX-209 (ethylene glycol monobutyl ether) and ARM (Urea/chloride complex) , while cleaning the concrete truck exteriors. Basic Ready Mix was previously cited for a violation of this occupational safety and health standard , which was contained in OSHA inspection number 314645565, citation number 1, item number 5 and was affirmed as a final order on December 06, 2011 with respect to a workplace located at 712 N. Wall Street, Calhoun, Georgia.
