Employees were exposed to amputations, and compound fractures while working near equipment that is not de-energized before conducting maintenance, changing belts, lubricating and replacing parts for equipment such as but not limited to, conveyors, aggregate bins, and the mixer.    Basic Ready Mix was previously cited for a violation of this occupational safety and health standard , which was contained in OSHA inspection number 314645565, citation number 1, item number 4 and was affirmed as a final order on December 06, 2011, with respect to a workplace located at 712 N. Wall Street, Calhoun, Georgia.
