29 CFR 1926.451(e)(6)(vi): Integral prefabricated scaffold access frames did not have a maximum spacing between rungs of 16 3/4 inches (43 cm). Non-uniform rung spacing caused by joining end frames together is allowed, provided the spacing does not exceed 16 3/4 inches (43 cm).  a) 6th and Delancey Street - Employees were not provided with a safe means of access and/or egress to the tubular welded frame scaffold platform, thus exposed to a fall hazard of approximately thirty-one (31) feet.  The spacing between rungs was approximately 21 inches.  Observed on or about August 27, 2013.
