29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph:    a)  Basement - On or about 08/14/2013, a wall mounted outlet box used to provided temporary power to a carpentry crew was not provided with a cover thereby exposing workers to 120 volt live parts.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:   a)  First Floor - Carpenters using a portable circular saw was not provided with an assured equipment grounding conductor program or ground fault circuit interrupters and were exposed to a 120 volt electrical shock hazard.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:   a)  First Floor - On or about 08/14/2013, a heavy-duty cord set used to power a portable circular saw had a missing ground pin and was, therefore, not grounded.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a)  Addition, First Floor - On or about 08/14/2013, carpenters were exposed to a 11 foot fall into an open basement for they had not been provided with fall protection while removing floor joists.      b)  First Floor, Rear Exterior Wall - On or about 08/14/2013, carpenters were exposed to an 11 ft. fall from the unguarded edge of an open-sided floor.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
