29 CFR 1910.178(l)(2)(iii): The employer did not ensure that operators training and evaluation was conducted by persons who have the knowledge, training, and experience to train powered industrail truck operators and evaluate their competence:   (a) Facility Wide - 28457 Ballard Rd. Unit G, Lake Forest, IL 60045 - The employer did not ensure that operators training and evaluation was conducted by persons who have the knowledge and training to train each operator of the electric powered walkie pallet trucks.    In accordance with 29CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.178(l)(6): The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:   (a) Facility Wide - 28457 Ballard Rd. Unit G, Lake Forest, IL 60045 - The employer did not provide a completed certification including the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation for each operator of the electric powered walkie pallet trucks.    In accordance with 29CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification.
