29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. a) Filling room area; the employer did not ensure that passageways were kept in good repairs. The concrete floor was uneven and cracked exposing employees to tip over/crushed by hazards while traveling over the uneven working/walking surface in a power industrial truck.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard. a) Outside storage tanks area; the employer did not ensure that the opened sided floors of the storage tanks C-1 and C-2 were equipped with a standard railing system. Employees were exposed to a fall of approximately 20 feet to the ground while engaged in monthly inspections at the very top of the storage tanks.
