29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening.   On or about 27 August 2013, and at times prior thereto, two employees would access a mezzanine area where items are stored in order to place or pull items from storage in which the ladderway opening to the mezzanine was not guarded, exposing the two employees to a fall hazard of approximately 107 inches or almost 9 feet to the concrete floor below.

29 CFR 1910.110(f)(6)(ii): Liquefied Petroleum Gas containers were not stored in a suitable enclosure or otherwise protected against tampering.   On or about 27 August 2013, and at times prior thereto, Liquefied Petroleum Gas tanks were being stored on a pallet within the yard of the facility without being stored and locked up in a cage, exposing the employees working there to both a fire and explosion hazard, and a struck by hazard from flying projectiles.
