29 CFR 1926.20(b)(2): Frequent and regular inspections of the job site, materials, and equipment to be conducted by a competent person designated by the employer was not provided by the employer who was responsible to initiate and maintain an accident prevention program:  a- 3 Kansas Road, Little Egg Harbor NJ: A competent person did not conduct frequent and regular inspections of the jobsite.  As such, employees at this site were exposed to hazards including, but not limited to struck by hazards.  Conditions noted on or about 07/02/13, 07/07/13, 07/10/13.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a- 3 Kansas Road, Little Egg Harbor NJ: Employees engaged in house lifting activities had not received safety instruction on the hazard(s) and regulation(s) applicable to their work.  Such training should include, but not necessarily limited to: recognition and control of hazard(s) posed by house lifting activiites, capacity/limitations of steel and other devices used to lift the structure, caught between hazards, struck by hazards, FEMA Engineering Principles and Practices for Retrofitting Flood-Prone Residentail Structures, etc.  Conditions noted on or about 07/02/13, 07/07/13, 07/10/13.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a- 3 Kansas Road, Little Egg Harbor NJ: Employees exposed to the possibility of head injury were observed working, without head protection, beneath and alongside steel beams, underneath and around raised structures, etc. when performing house raising and other related activiites at the worksite.  Conditions noted on or about 07/02/13, 07/07/13, 07/10/13.   NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
