29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a) For the Ven-Tel facility at 11311 74th Street North, Largo, FL 33773, as observed on or about 08/27/2013, employees were exposed to unguarded sections of unused blade on a horizontal metal bandsaw. On the left side of the saw, the unused blade was measured to be 8.5 inches long, and on the right side of the saw, the unused blade was measured to be 5.5 inches long. Employees operating the saw were exposed to potential laceration hazards due to the unguarded saw blade.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):   For the Ven-Tel facility at 11311 74th Street North, Largo, FL 33773, as observed on or about 08/27/2013, lockout tagout procedures were only general in nature and not specific to the machinery on which lockout tagout was performed, in that:   a) Specific steps on the application of locks or tags to machine specific energy isolating sources were not present.   b) Specific procedures for testing a machine or equipment to determine and verify effectiveness of lockout tagout devices and other energy control measures were not present.   c) Procedural steps for shutting down and isolating machinery and equipment did not all identify what the energy isolating sources were and where they were located.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:   For the Ven-Tel facility at 11311 74th Street North, Largo, FL 33773, as observed on or about 08/27/2013, tool rests on abrasive wheel bench grinders were not adjusted to have a gap between the tool rest and abrasive wheel of 1/8 inches (0.125-inches) or less:          a) In the maintenance shop, the gap between tool rest and abrasive wheel was 1.125 inches.       b) In the tool/die cleaning room, the gap between tool rest and abrasive wheel was 0.31 inches.

29 CFR 1910.215(b)(9):  The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch (0.25):      For the Ven-Tel facility at 11311 74th Street North, Largo, FL 33773, as observed on or about 08/27/2013: tongue guards on multiple bench grinders were missing, and gaps between the abrasive wheel periphery and safety peripheral member were greater than ¼ inches:      a) In the maintenance shop, the gap between the abrasive wheel and safety peripheral member was 0.94 inches.   b) In the tool/die cleaning room, the gap between the abrasive wheel and safety peripheral member was 0.94 inches.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:      a) For the Ven-Tel facility at 11311 74th Street North, Largo, FL 33773, as observed on or about 08/27/2013, a power cord for a lathe had its outer insulation pulled away exposing inner wiring and placing strain on inner joints and terminal screws.
