29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) For the Ven-Tel facility at 11311 74th Street North, Largo, FL 33773, as observed on or about 08/27/2013, employees were exposed to unguarded sections of unused blade on a horizontal metal bandsaw. On the left side of the saw, the unused blade was measured to be 8.5 inches long, and on the right side of the saw, the unused blade was measured to be 5.5 inches long. Employees operating the saw were exposed to potential laceration hazards due to the unguarded saw blade.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): For the Ven-Tel facility at 11311 74th Street North, Largo, FL 33773, as observed on or about 08/27/2013, lockout tagout procedures were only general in nature and not specific to the machinery on which lockout tagout was performed, in that: a) Specific steps on the application of locks or tags to machine specific energy isolating sources were not present. b) Specific procedures for testing a machine or equipment to determine and verify effectiveness of lockout tagout devices and other energy control measures were not present. c) Procedural steps for shutting down and isolating machinery and equipment did not all identify what the energy isolating sources were and where they were located.
