OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a) On or about August 27, 2013, and at times prior, employees were exposed to crushing and struck-by injuries in that at least eight out of twenty eight automotive lifts at this location did not have posted information related to the load capacity. Employees were confused as to the correct load capacity of their lifts. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a) On or about August 27, 2013, and at times prior, employer did not ensure that cables were connected to devices and fittings so that strain relief was provided to prevent pull from being directly transmitted to the joints or terminal screws. An electrical cable connecting to the motor for automotive lift #23 showed signs of strain, the outer insulation at the fitting was damaged, exposing the live inner wire insulation.
