29 CFR 1926.304(f): Section 4.1.2(c) American National Standards Institute 01.1-1961, Safety Code for Woodworking Machinery as adopted by 29 CFR 1926.304(f): Circular hand-fed rip saws did not have non-kickback fingers on dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator: The employer failed to protect employees from potential struck by or impalement hazards associated with operating a table saw. The most recent example of this was found on the job site located at 1800 Fletcher Ave, Lincoln, NE 68506 where the employer had an employee using the Rigid table saw without kickback feet installed; thus exposing employees to the potential of being struck by the wood they are cutting in the event the cutting blade binds.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The employer failed to protect employees from potential fall hazards associated with operating an aerial lift without adequate fall protection. The most recent example of this was found on the job site located at 1800 Fletcher Ave, Lincoln, NE 68506 where the employer had an employee siding a new residential townhome while working out of an aerial lift with inadequate fall protection. Instances of inadequacy include but are not limited to: i. The employee was wearing the fall arrest harness improperly, specifically did not have the legs straps wrapped around his legs. ii. The employee was wearing the fall arrest harness without having secured a lanyard to his harness and the appropriate anchor point in the bucket.
