29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:      The employer does not ensure that each employee on a walking/working surface with an unprotected side or edge which is 6 feet or more above a lower level is protected from falling.  This violation was observed on or about August 28, 2013, on the northeast side of the retail center where employees were exposed to a fall hazard greater than 6 feet when installing wiring for security cameras without a fall protection system.        Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each employee on a walking/working surface with an unprotected side or edge which is 6 feet or more above a lower level is protected from falling.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:       The employer does not ensure that when a portable ladder is used for access to an upper landing surface, the ladder side rails extend at least 3 feet above the upper landing surface to which the ladder is used to gain access.  This violation was observed on or about August 28, 2013, on the northeast side of the retail center where employees were exposed to a fall hazard when using an extension ladder to access a roof to install wiring to security cameras without extending the ladder 3 feet beyond the landing of the roof.          Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that when a portable ladder is used for access to an upper landing surface, the ladder side rails extend at least 3 feet above the upper landing surface to which the ladder is used to gain access.
