On or about September 5, 2013, at the production area-PW33 Case packer of the Sinchro 4.0, of the above addressed jobsite, the employer failed to develop a specific energy control procedure for the control of multiple energy sources as applicable for an authorized employee to clean a machine, exposing the employee to a caught by hazard.      c.  On or about October 15, 2013, at the robot's area, of the above addressed worksite, the employer failed to develop a specific energy control procedure for the control of multiple energy sources as applicable for an authorized employee to perform servicing of the robots, exposing the employee to caught by and struck by hazards.

On or about September 5, 2013, at the converting mechanic shop of the above addressed worksite, an employee was exposed to a struck by hazard while working with a bench grinder (i.e. Dayton, Model # 4Z9909C) had its right side tongue guard resting over the peripheral guard, and as a result separated more than 1/4-inch from the wheel.
