29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    (a) Location: 39 Irving Street, Worcester, MA: On or about 28 August, 2013 one employee was observed performing demolition work by the edge of a roof approximately 40 feet above the ground below without any fall protection as required by this subpart:    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   Location: 39 Irving Street, Worcester, MA: On or about August 28, 2013 an employee performing roofing work was exposed to falls up to 40 feet and had not been provided fall hazard training in accordance with this subpart.
