29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On August 28, 2013, an employee was not protected by a hard hat while being exposed to overhead hazards, exposing the employee to a struck-by hazard. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(g)(1): 29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: On August 28, 2013, an employee was not protected from a fall while working from a scaffold, exposing the employee to a fall of 12.5 feet. The W.M Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 617738, citation number 2, item number 2 and was affirmed as a final order on October 16, 2012, with respect to a workplace located at 4116 N. Leavitt St. Chicago, IL 60618. The W.M Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 312598162, citation number 2, item number 1 and was affirmed as a final order on December 2, 2009, with respect to a workplace located at Wolf & McCarthy Rd. Palos Park, IL 60464. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
