29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) BP Electric of Ohio on or about August 28, 2013, at the address of 731 W. Trenton Ave. Findlay, OH: The employer failed to ensure employees installing lighting fixtures atop a commercial structure were protected from a fall. The employees were exposed to an 18 ft. fall to the concrete below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) BP Electric of Ohio on or about August 28, 2013, at the address of 731 W. Trenton Ave. Findlay, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing electrical wiring working on top a commercial structure.
