29 CFR 1910.119(e)(6): The process hazard analysis was not updated and revalidated every five years: a) Severn Trent Services Inc., Downingtown Water Pollution Control Center- The employer performed an initial Process Hazard Analysis (PHA) in May, 1997 for the post-disinfection process utilizing chlorine gas to form a chlorine gas water solution for chlorine contact disinfection of the plant effluent. There was no update or revalidation of this initial PHA performed for this process. Condition observed on or about August 30, 2013. Note: Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
