29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead; This violation was discovered on or about September 26, 2013, when wipe sample results taken in the break room area of BBM Inc. indicated the presence of lead on table tops, refrigerators, microwaves, and countertops. Wipe samples further showed lead contamination on ungloved hands of several employees.

29 CFR 1910.1025(m)(1)(iii): Employers shall include lead in the hazard communication program established to comply with the HCS (� 1910.1200). Employers shall ensure that each employee has access to labels on containers of lead and to safety data sheets, and is trained in accordance with the requirements of HCS and paragraph (l) of this section. This violation occurred on or about August 28, 2013, where employees who were potentially exposed to eye or skin contact with lead were not provided training in accordance with the requirements of the lead standard, 29 CFR 1910.1025, or the hazard communication standard, 29 CFR 1910.1200.
