29 CFR 1910.107(b)(3):   The floor surface of a spray booth and operator's working area, if combustible, were not covered iwth non-combustible material of such character as to facilitate the safe cleaning and removal of residues:    On or about August 28, 2013, and times prior thereto, a table in the spray booth on the west side of building was covered with craft paper.  Employees were exposed to the hazards associated with fire.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:   On or about August 28, 2013, and times prior thereto, the employer did not develop and implement a written respiratory protection program for employees required to wear tight fitting half face respirators.  Employees were using plating and coating materials that contained chemicals such as, but not limited to, potassium pyrophosphate in concentrations of 40-50%, sodium carbonate in concentrations of 20-30%, sodium hydroxide in concentrations of 10-20%, ethyl benzene at concentrations of 5-10%, xylenes (o-,m-,p-isomers) at concentrations of 30-40%, and toluene at concentrations of 30-60%.  Employees were exposed to the hazards associated with chemical inhalation and exposure.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  On or about August 28, 2013, and times prior thereto, the employer did not provide facilities for quick drenching or flushing of eyes and body for employees using corrosive chemicals such as, but not limited to, potassium pyrophosphate in concentrations of 40-50%, sodium carbonate in concentrations of 20-30%, and sodium hydroxide in concentrations of 10-20%.  Employees were exposed to the hazard of eye and skin injuries from contact with corrosive chemicals.
