29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: (a) Toyota model 42-6FGC425 Serial 80724 fork lift was used with tires in poor condition to include missing chuncks, splits and separation from the rim. (b) Toyota model 42-6FGC425 Serial 80724 fork lift was used with horn in inoperable condition. (c) Toyota model 7FGCU25 Serial 99421 was used with tires in poor condition to include missing chuncks, splits and separation from the rim. (d) Toyota model 7FGCU25 Serial 99421 was used with the flashing indicator in inoperable condition and the cover broken.
