29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  Country Repairs did not provide certification on each of the cited items:  (a) The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Items 2a, 2b, and 2c, for hazards associated with:  (1) Failure to establish and implement a hazard communication program for employees who work with chemicals, including but not limited to, JM EPDM membrane adhesive, solvents, and other adhesives while installing roofing materials,  (2) Failure to keep safety data sheets in the workplace for employees who work with the chemicals identified in (1) above,  (3) Failure to provide effective information and training on hazardous chemicals for employees who work with the chemicals identified in (1) above.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a) On or about August 30, 2013 at the worksite; employees were installing the metal trim bar on the edge of the parapet wall, approximately 2 ft. tall, on a low slope roof at heights over 6 ft. without using any type of conventional fall protection or safety monitor.          b) On or about August 30, 2013 at the worksite; employees had to retrieve roofing materials that were staged on the edge of the parapet wall, approximately 2 ft. tall,  on a low slope roof at heights over 6 ft. without using any type of conventional fall protection or safety monitor         c) On or about August 30, 2013 at the worksite; employees were removing and installing rubber roofing material in the center of low slope roof, over 50 ft. wide, without having any type of warning line system or safety monitors being utilized.

As of August 30, 2013, the employer failed to establish and implement a hazard communication program for employees who work with chemicals, including but not limited to, JM EPDM membrane adhesive, solvents, and other adhesives while installing roofing materials.
